4. Sign in using CU Denver credentials
5. There will be two waivers! Scroll to the bottom of each, click the gray box, and sign.

Member Agreement

i.        Membership to Lola & Rob Salazar Student Wellness Center (LRS-SWC) is non-refundable / non-transferable.

ii.       Cancellation of electronic draft and payroll deduction based services requires a 30-day cancellation notice. All cancellations must be initiated through filling out our Membership Cancellation Form: https://ucdenverdata.formstack.com/forms/membership_cancellation_form

iii.      Cancellation/refund of pre-paid services may be granted only for one of the following reasons with required documentation:

·         Medical emergencies that inhibit the member from utilizing the SWC

·         Relocation

·         No longer employed at CU Denver

CU Denver Students, faculty, and staff are required to wear their badges at all times when on campus in an effort to ensure a safer and more secure campus. Beginning January 1st, 2024, The Wellness Center will require every member to have their Lynx Card or Wellness & Recreation Membership Card to enter the facility. If you do not have your university ID, you will not be admitted into the Wellness Center – no exceptions. University policy also restricts the use of CU Denver ID cards to their owner; therefore, students may not lend their IDs to anyone else, including other students.
